TMJ Pain and bruxism management

Temporomandibular Joint Pain Dysfunction Syndrome (TMJPDS) is one of the most common causes of jaw joint problems. 

It may involve:

  • The jaw joint itself
  • Muscles of mastication (chewing)
  • Myofascial pain (muscle-related discomfort)


These issues can usually be effectively managed with your usual dentist — with no need for referral in most cases.


Common TMJPDS symptoms 

Patients often experience:

  • Dull ache in or around the ear (can radiate to cheekbone, neck, or temple)
  • Joint noise (clicking, cracking, crunching, or popping)
  • Limited mouth opening
  • Headaches (especially in the temples)
  • Mild facial swelling, especially in the morning

Symptoms often worsen with chewing or during periods of stress.

Bruxism is a broad term used to describe a group of jaw muscle activities that include:

  • Teeth grinding
  • Teeth clenching
  • Bracing or thrusting of the lower jaw 

Bruxism can occur:

  • During sleep (sleep bruxism)
  • While awake (awake bruxism)


Bruxism is not simply a dental issue — it’s often driven by central (neurological and psychological) factors, such as:

  • Stress sensitivity
  • Emotional states
  • Personality traits
  • Sleep disturbances
  • Autonomic nervous system regulation

We follow evidence-based strategies to help patients manage TMJPDS effectively:

  1. Patient education – ensuring you understand the condition 
  2. Reassurance – symptoms often improve with simple steps.
  3. Heat therapy – warm compresses (e.g., hot water bottle wrapped in a towel) applied to the side of the face.
  4. Pain relief – NSAIDs like ibuprofen (if not contraindicated), taken regularly for 2–3 weeks, not just when needed. Topical NSAID gels can also help.
  5. Avoiding harmful habits – address clenching, grinding, nail biting, lip/cheek chewing, and poor jaw posture. 
  6. Diet modification – a soft diet may be advised, especially during flare-ups.
  7. Stress awareness – recognising links between symptoms and emotional stress or anxiety.
  8. Jaw exercises – this can help alleviate any stress or strain on your jaw muscles, the NHS provides free jaw exercises that you can do at home here.
  9. Occlusal splints – night-time soft splints (made by a dentist) are beneficial for patients who grind their teeth at night, and are usually used in conjunction with the above points.

Bruxism doesn’t always require treatment, but when it leads to symptoms or damage, it may result in:

  • Tooth wear or cracked teeth 
  • Failure of dental restorations or implants
  • Jaw muscle pain, fatigue, or enlargement
  • Temporal (side of head) headaches
  • Toothache symptoms
  • Temporomandibular disorders (TMD)
  • Sleep disturbance (especially for bed partners)
  • Overall reduced quality of life


Treatment focuses on the impact rather than the presence of bruxism itself. If it is clinically significant, we may recommend:

  • Counselling and education – understanding triggers and reducing contributing stress
  • Custom oral appliances – detailed below in the advanced treatment section
  • Cognitive-behavioural techniques – for stress and habit control
  • Physiotherapy – targeted exercises and jaw relaxation therapy (we can recommend Cathy Gordon.
  • Short-term muscle relaxants – used rarely and only if appropriate

  • Holds the lower jaw slightly forward to maintain an open airway during sleep.
  • Reduces both grinding activity and sleep-disordered breathing.

  • Used in patients with internal TMJ disc displacement or joint clicking.

Botulinum toxin can be a powerful tool for managing chronic bruxism or TMJ pain caused by muscle hyperactivity.

  • Administered into the masseter muscles (jaw muscles involved in chewing and clenching).
  • Helps reduce clenching force, soften jaw tension, and prevent further tooth damage.
  • Treatment typically takes 15–20 minutes and lasts 3–6 months.
  • Particularly beneficial when other methods have failed or symptoms are severe.
