Mobile IV sedation Referral form

Before requesting sedation services in house, please ensure the following requirements are met:  

• You have read the advice and understood the instructions on the 'Join Us' page.
• Your practice is fully compliant with CQC sedation guidance and IACSD standards. 
• The team members working with sedated patients have in-date ILS (immediate life support) certification.
• The practice has a comprehensive sedation policy, which includes the techniques and age groups requested in the referral. 

N.B. We politely request that patients be informed that our team's provision of sedation is subject to satisfactory pre-operative assessment. Please do not book dates for sedation and then retrospectively request a sedationist. We cannot confirm sedation can be provided until the above criteria are met.
